Blepharoplasty, also known as an eyelid lift, literally means shaping the eyelids and is derived from the Greek words blepharon, or eyelid and plastikos, to mold or to shape. This is one of the most sought after procedures in plastic surgery.
A blepharoplasty is a safe and effective procedure with a short recovery period that delivers long lasting results. Our Craniofacial Plastic Surgeon, Dr. Costa, is an specialist in the art of blepharoplasty surgery. Dr. Costa finds eyelid surgery very rewarding because it yields dramatic results with a rapid recovery for her patients that will last them many, many years to come. Dr. Costa performs aesthetic eyelid lift surgery for patients in San Jose, the San Francisco Bay Area and South Bay, California, and outside California.
Eyelid Surgery in the San Francisco Bay Area
As you age, your eyelids stretch, and the supporting muscles and tissue weaken. As a result, excess fat may gather above and below your eyelids, causing sagging eyebrows, droopy upper lids and bags under your eyes.
In addition to an older appearance, severely sagging skin around your eyes can reduce your peripheral vision (side vision), especially the upper and outer parts of your field of vision. Blepharoplasty can reduce or eliminate these vision problems and make your eyes appear younger and more alert.
Eyelid rejuvenation, or surgery for the eyelids, may be an option if you have a saggy or droopy upper eyelid, excess skin of the upper eyelids that interferes with your peripheral vision, excess skin on the lower eyelids or bags under your eyes.
Blepharoplasty is a favorite procedure among our patients in part due to the large improvements and faster recovery than other procedures.
Surgery for Eyelids: Types of Eyelid Lifts in the South Bay
If you are considering eyelid surgery in the South Bay, the following will help you become familiar with the different options available.
Upper Eyelid Lift
An upper eyelid lift, or upper blepharoplasty, will remove excess skin of the upper eyelid. Any excess fat is removed as well.
In some cases, fat is added, or grafted, to the upper eyelid region. In Asian blepharoplasty, a new supratarsal, eyelid, and fold is created. Incisions are hidden in natural skin creases.
Ptosis Surgery for Droopy Eyelids
In some cases, the muscles that lift the upper eyelid do not work properly. The eyelid droops and requires muscle repair. Dr. Costa performs ptosis repair in San Jose to address this problem.
Lower Eyelid Lift, Under Eye Lid Surgery
In a lower eyelid lift, or lower eye blepharoplasty, depending on your needs, Dr. Costa may reduce the fat protruding under your eyes, remove excess skin under your eyes, and fill in the under eye hollows. This filling in can be done with your own fat or with an injectable filler.
Double Eyelid Surgery in the Bay Area
What is an Asian Blepharoplasty? About half of individuals with an Asian eyelid will have a defined eyelid crease. The presence of this crease is also known as having a double eyelid. This refers to the part of the eyelid above and the part below the eyelid crease. Some individuals who do not have a crease would like to have an Asian blepharoplasty to create a fold. The goal that most patients desire is a fold that honors the integrity and appearance of the eyelid and maintains the natural Asian appearance. Double eyelid surgery is the most common aesthetic procedure performed in Asian males and females. Dr. Costa prefers the incisional technique for Asian eyelid surgery in the Bay Area. This hides the scar in the fold and the results are reliable and long lasting. The suture only technique does not require an incision to be made and has a faster recovery; however, the results do not last as long and are not as reliable.
Canthopexy and Canthoplasty
These are lower lid tightening procedures that may be needed if the ligament supporting your lower lid, the lateral canthal fold tendon, is weak, or loose.
A quad, or four lid blepharoplasty, is when eyelid surgery is performed on the upper and lower lids at the same time. This is a very effective approach to rejuvenation around the eyes. It is also quite practical, as both the upper and lower lids tend to show signs of aging together. Dr. Costa performs this procedure often and often combines it with nano fat grafting to the eyelid hollows to restore volume to these areas.
*A Brow Lift is not an eyelid lift. However, the eyebrows and the upper eyelids are closely related. Many individuals have drooping brows and this affects the appearance of their eyes. If you feel this is you, you may benefit from a brow lift. Dr. Costa will assess whether you would benefit from this during your consultation.
Plastic Surgery for the Eyes: What are the Benefits of an Eyelid Lift in San Jose California?
A blepharoplasty results in a naturally more youthful, rested appearance.
Upper and lower eyelid rejuvenation help patients to look and feel better. Blepharoplasty provides a refreshed look to the upper face.
Nano fat grafting and laser skin resurfacing provide a complete transformation by filling in the hollows and reducing lines and leaving you with a soft, natural result.
Upper Blepharoplasty (Aesthetic Eyelid Surgery) in the Bay Area
If you feel that you have heavy, full, or drooping upper eyelids, you may benefit from a blepharoplasty.
What is an Upper Blepharoplasty or Upper Eyelid Surgery Lift?
The skin above the upper eyelid can become redundant and excessive. These changes produce a tired appearance. An upper eyelid lift is a simple procedure that restores a more youthful, rested, and brightened appearance to the upper eyelid area.
Surgery for Tired Eyes: What Does an Upper Eyelid Lift Do?
An upper eyelid plastic surgery rejuvenates the upper eyes and provides long term changes. Upper eyelid surgery lifts the upper eyelids by removing a carefully calibrated amount of excess skin and a small strip of muscle to contour your eyelid. If you have puffy eyes from bulging fat, some will be removed. Dr. Costa does not remove too much fat because that can result in a hollowed, aged appearance.
Treatment Goals in an Upper Blepharoplasty
The goals in an upper blepharoplasty are customized to each patient. In general, the goal is to produce a refreshed, rested appearance to the eyelids. Dr. Costa creates a soft contour which is associated with a more youthful appearance. Only removing the amount of fat that is needed to treat bulges avoids an over operated appearance.
Precision Bay Area Eyelid Surgery
In our Bay Area Blepharoplasty, all scars are hidden. Dr. Costa’s technique places the incision precisely where it needs to go to give a narrow, nearly undetectable scar. Dr. Costa conceals the incision in your natural eyelid crease. This scar heals incredibly well.
Removing Fat from the Upper Eyelids
If there is excess fat, Dr. Costa removes fat from the eyelids judiciously, only removing eyelid fat that is needed to correct bulging and puffiness.
Rejuvenated, Natural, Beautiful Results
Dr. Costa’s natural appearing results often cause family and friends to notice how refreshed you look, not knowing that you had any surgery. Depending on your individual needs, additional goals are to eliminate any obstruction to vision caused by drooping skin and reduce the feeling of heavy lids caused by excess upper lid skin. Also, if you have hollows in the upper eyelid region, this is addressed by treating the hollowed area with fat grafting.
What are the Benefits of an Upper Eyelid Lift in San Jose California?
Eyelid rejuvenation offers aesthetic and functional benefits.
An upper blepharoplasty is a simple yet transformative procedure – it produces clear improvements in the aesthetics of the eyes and brow creating a dramatically more youthful appearance. Eyelid rejuvenation also offers functional benefits. An upper blepharoplasty can improve vision by removing any obstructions from excess skin.
The upper eyelid lift is a powerful, long-lasting procedure. Your results will last for over a decade. It is a reliable procedure that consistently produces the desired result.

Changes to the Upper Eyelid Area
Extra Eyelid Skin and Hooding
With age, extra skin accumulates on the upper eyelids. This can result in an eye roll from the excess eyelid skin. When the extra skin builds up even further and reaches the lash line, this is called hooding. Lateral hooding appears as excess skin obviously affecting the outer part of the lids causing a hooded effect. Removing extra eyelid skin, or eye skin removal corrects this problem.
Difficulty Applying Makeup
The upper eyelid skin may in some cases reach down to the upper eyelash line. The excess skin can interfere with the application of makeup and can be uncomfortable causing the eyelids to feel heavy.
The fat of the upper lid may become more prominent with time. This causes puffy eyelids from the bulging fat. The fat is normally held in place by tissue in the area. When this tissue weakens, the fat can spill forward. In this case, removal or repositioning of fat is performed to correct this problem.
As the skin sags, the upper eyes can appear more closed, like slits. A youthful eye shape is round and open.
Visual Obstruction, Difficulty Seeing
If the excess skin is more severe, it can even impact vision. When the excess skin is so much that it hangs down or past to the lashes and causes hooding, it may block the visual fields in the upper and outer side areas. A Functional Blepharoplasty improves vision by removing the hooded skin blocking your upper visual field.
Naturally Full Upper Eyelids
Upper blepharoplasty is not only for people with changes related to age. Some individuals have a naturally full upper eyelid and may desire to change this.
Early Onset Excess Eyelid Skin
Some people wonder if they are too young to have upper eyelid surgery. The reality is that for some people, the development of extra eyelid skin and even hooding begins at a much younger age. We have had patients tell Dr. Costa that this runs in their family. If you have excess upper lid skin, even if you are younger, we can help.
Wrinkles around the Eyes
Periocular rhytids, or crows feet, are fine lines and wrinkles around the eyes that occur with age. These are best treated with skin revitalization with laser skin resurfacing and microneedling and with a neurotoxin such as Botox®. The skin resurfacing will help to remove the fine lines and wrinkles. The neurotoxin such as Botox will help prevent lines from forming in the future and will soften existing lines.
In some cases, the brows are droopy. A sagging eyebrow will contribute to the upper eye area appearing heavy and older. If your brows are droopy, this will be taken into consideration as part of your consultation. A Brow Lift may be recommended to achieve complete rejuvenation of your upper face.
Upper blepharoplasty, also known as an upper eyelid lift, repairs droopy eyelids and involves removing excess skin, typically with a small amount of muscle and possibly fat removal from the eyelid or repositioning of the fat.
Male Eyelid Surgery in San Jose California
Male blepharoplasty, or eyelid surgery for men is a sought after procedure in the San Francisco Bay Area. Both upper and lower eyelid lifts in men produce natural, refreshed results that last for well over a decade. In many cases, a male eyelid lift can improve your sight. Men tend to have eyelid skin that becomes more puffy and heavy with time. An eyelid lift can correct this and help you to look less tired. Lower eyelid surgery, often requested as eye bag surgery for men, treats the under eye puffiness and any extra lid skin.
Lower Blepharoplasty in San Jose California
There are different types of lower blepharoplasty and different techniques to address the different variables related to the lower eyelid and to aging. The anatomy and issues that arise in the lower eyelid are different from the upper eyelid area. Lower eyelid lifts are quite different from upper eyelid lifts. Because of this, different anatomic considerations and treatment techniques must be taken into account.
What is a Lower Blepharoplasty or Lower Eyelid Lift?
A lower blepharoplasty is an aesthetic procedure that restores and rejuvenates the lower eyelid region. It is a transformative, outpatient procedure that helps to correct excess skin, under-eye bags, and dark circles.
What Does a Lower Eye Lift Do?
Lower eyelid surgery, also known as eye bag surgery, treats under eye bags and excess skin that cause a tired, more aged appearance. Over time, the lower eyelid skin can become redundant and excessive. These changes produce a tired appearance. Under eye bags from bulging fat can occur. Hollowing in the area and dark circles, or a dark discoloration can occur. A lower eyelid lift restores a more youthful, rested, and brightened appearance to the lower eyelid area.
Excess skin is removed with an incision under the lash line.
Under eye bags are reduced with an incision inside the eye lid.
Nano fat, or micro fat, grafting volumizes the hollows around the eyes.
Fine lines and under eye wrinkles are reduced with skin resurfacing.

Differences Related to Lower Eyelid Appearance
Lower Eyelid Bags
Bags under the eyes can cause individuals to feel that they look tired when they are not. Our patients with under eye bags often complain that they don’t feel as old as the bags make them look. The appearance of eyelid bags can be related to various factors:
Herniation of orbital fat, or bulging fat: The delicate tissues that typically keep the periorbital fat, or fat around the eye, in position, become weak. When this tissue weakens, the fat spills forward. This fat herniation causes the appearance of bags.
Loss of Volume
Loss of volume of the mid-face: As volume is lost in the area between the eyelid and cheek, it can result in a more aged appearance.
Drooping of the Tissues around the Eyes
Mid-facial descent: With time, the soft tissues around the cheek, or mid-face, begin to lose support and move downward. This descent causes a hollowing in the lower eyelid area. This hollowing accentuates the appearance of any under-eye bags.
The lower eyelid skin can become excessive with time. These changes can be related to factors such as sunlight, or UV radiation exposure, cigarette smoking, and rubbing the under eye area. In addition, over time, the delicate lower eyelid skin can become thinner.
There are multiple causes for dark circles. These including the anatomy between the eye and the cheek, allergies, lack of sleep, eyelid bags causing shadowing, and eyelid skin quality and integrity.
Customized Comprehensive Treatment for Eyelid Rejuvenation in the San Francisco Bay Area
Dr. Melinda Costa treats the upper and lower lower lid areas with a multifaceted, customized approach at her practice in San Jose.
The main components of eyelid rejuvenation involve removing excess skin, volumizing any hollows with nano fat or micro fat grafting, removing any eyelid bags, resurfacing the lids to reduce lines with periorbital resurfacing, softening and preventing fine lines and wrinkles with Neuromodulators.
Plastic Surgery for Baggy Eyes – Removing Fat and Extra Skin
Any bulging under eye fat, excess skin, hollowing in the under eye area, or fines lines and wrinkles are systematically addressed and treated. A lower blepharoplasty, or cosmetic surgery for puffy eyes, will reduce the bags and redrape the skin to leave a firmer appearance. Eye fat removal surgery is done through an incision hidden inside the eyelid.
Restoring Volume Naturally to the Eyelids and Midface
Nanofat grafting treats the under eye hollows and any hollowing in the upper lids. This precise, technical procedure smoothes the transition between the lower lid and cheek area and provides long lasting, natural results.
Treating Fine Lines and Wrinkles: Resurfacing and Neuromodulation
The addition of laser resurfacing reduces skin’s fine lines and wrinkles around the eyes. Neurotoxin (Botox®) treatment prevents the formation of fine lines and wrinkles and softens the existing ones. The result of this systematic, comprehensive approach is a rejuvenated, natural appearing eye area with long lasting results.

Types of Operations for Lower Eyelid Surgery
Transconjunctival Lower Blepharoplasty
In this form of a lower eyelid lift, an incision is made on the inside of the lower eyelid. This is a hidden incision that will not be visible. Excess fat around the eye is removed from this incision. This approach to blepharoplasty is useful in removing or repositioning under eye bags and is also known as a fat mobilization blepharoplasty or removal of eyelid fat. If there is a small amount of excess skin, a pinch blepharoplasty (see below) can be done at the same time to remove excess skin.
Laser skin resurfacing can also be done to address fine lines and wrinkles and excess lower lid skin.
Transcutaneous Lower Blepharoplasty
A transcutaneous lower eyelid lift is done by making an incision in the eyelid skin. The incision is placed just below the eyelash line. This type of eyelid lift is useful when there is excess skin to be removed.
Pinch Lower Blepharoplasty
A pinch lower blepharoplasty, or skin pinch eyelid lift, is a type of transcutaneous blepharoplasty. A small amount, or pinch, of skin, is removed. The scar is hidden just under the lash line and is nearly invisivle once healed. A pinch eyelid lift is useful for individuals with a small amount of excess skin or as a touch up. Skin resurfacing with a laser or peel can be done at the same time or a later time.
Eyelid Lift in the Bay Area Surgical Treatment: The Operation
Dr. Costa ensures the best results for each patient and no procedure is ever rushed.
A plastic surgery eyelid lift creates a refreshed more youthful appearance. Lower eyelid lifts are often done at the same time as an upper eyelid lift.
What is the Upper Eyelid Lift Operation like?
The upper eyelid lift can be done safely in the office under local anesthesia. It may also be done in the operating room, particularly if it is combined with other procedures. Our patients often combine an upper eyelid lift with other facial rejuvenating procedures such as a lower blepharoplasty, nanofat grafting and a face and neck lift. The upper eyelid lift is generally done as an outpatient procedure. This means you will go home the same day, unless combining the procedure as part of a larger operation, in which case you may stay overnight,
The procedure takes one hour. Dr. Costa carefully marks the pattern of skin to be removed. After you are numb from anesthesia, the excess skin on your upper eyelid is removed. The skin amount and pattern removed is carefully calibrated by Dr. Costa immediately prior to the blepharoplasty. Dr. Costa removes a small strip of muscle in order to create a pleasing contour at your lid crease. Dr. Costa takes diligent care to remove the right amount of skin to achieve the goal that you and Dr. Costa agree upon during the pre-surgical process. Ultimately, your eyes are transformed into a pleasing, more youthful appearance.
What is the Lower Eyelid Lift Operation like?
Dr. Costa typically performs the lower eyelid lift in the operating room for safety and comfort. The procedure, if combined with nano fat grafting, typically takes 1 1/2 to 2 hours. Dr. Costa wants every patient to have the best results. No patient is ever rushed.
If under eye bags are to be removed, Dr. Costa performs this with a hidden incision inside your lower eyelid (transconjuntival approach). If skin is to be removed, Dr. Costa removes the excess skin with an incision hidden just below your eyelashes (transcutaneous approach). For any under eye hollows, nano fat grafting is done to carefully and precisely restore the volume to these areas. All of these procedures may be done, depending on your individual needs.
If you have fine lines and wrinkles in the area, laser resurfacing is done postoperatively after the initial swelling and bruising have subsided.
What Type of Anesthesia is Used in a Blepharoplasty?
An upper blepharoplasty and in some cases, a lower blepharoplasty can be performed in the office under local anesthesia. Numbing medicine is placed in the area. If you are having the procedure in an operating room, sedation or general anesthesia may be given.
Lower blepharoplasty is often performed with local anesthesia and sedation. If combined with other procedures, general anesthesia may be used.
Eyelid Enhancement Recovery and Aftercare
The recovery tends to be fairly rapid and we work with our patients to make it as smooth as possible and minimize downtime.
There is some initial swelling and bruising that starts right after and are present for a few days postop. Swelling will peak at 3 days (72 hours) and will improve after.
Sutures are removed in 5-7 days.
It is normal to have dry or irritated eyes for the first week. It is normal to experience blurriness for a few days. Most swelling and bruising resolves in 10 to 14 days.
It is normal to have a little asymmetry of the eye lids until the swelling improves.
Activities that don’t require exertion can be resumed the very next day. Most patients return to work within 5 to 7 days and resume full activities in 2-3 weeks.
Icing the area for the first 48 hours can be very helpful and is recommended to maximize comfort and speed recovery. Elevating your head on an extra pillow or two for 1-2 weeks will help to minimize swelling and help you feel more comfortable. Arnica can be used to reduce swelling and bruising. Sutures are removed 5 days after the procedure. Coverup and makeup can be worn the day following suture removal. No dressing is needed, and antibiotic ointment is used over the suture line under the sutures are removed. Avoid strenuous activity for one week.
What is the Scar from a Blepharoplasty Like?
Is there a scarless blepharoplasty? When performing the upper eyelid lift, Dr. Costa carefully places the scar precisely at the natural eyelid crease. In the way, the incision is hidden in this natural crease. As the scar matures with time, this approach allows the scar to be virtually indetectable. The scar may be pink early in the healing process and can be easily concealed with makeup. In addition, scars in this area tend to heal well. Due to Dr. Costa’s focus on and experience with facial aesthetics and reconstruction as a Craniofacial Surgeon, you can be assured you will have an expert focusing on your eyes.
In the lower blepharoplasty, Dr. Costa places the incision inside the lower eyelid, which hides the scar. In order to redrape the lower lid skin to eliminate the excess skin, Dr. Costa places the scar immediately below the natural eyelash line, where it conceals extremely well.
In summary, blepharoplasty scarring is quite minimal and barely noticeable when done by an experienced, meticulous surgeon.
An example of blepharoplasty scars in the photo below is seen:
How Will a Blepharoplasty Change My Appearance?
An upper and lower eyelid lift are designed to help restore a more youthful and natural appearance. Dr. Costa will perform the operation to turn back the hands of time without changing your individual appearance. You will look like yourself, refreshed. The goal is for those who know you to feel that you look different somehow, as if you have gotten more sleep or have been on vacation and not to know that you had surgery.
Will an Upper Blepharoplasty Make it Easier to Apply Make Up?
Yes, it will! Many patients are bothered by the difficulty they have in applying make up due to excess upper eyelid skin. The makeup tends to smear and is rather a nuisance. Our patients tell us how happy they are after our San Jose eyelid lift not only for how they look but for the practical benefits to being able to apply makeup without smearing.
Do I Need Surgery on my Eyebrows?
Possibly. The eyebrows can droop with time. When the eyebrows droop, this creates a more tired and aged appearance. Sagging eyebrows will bunch up the skin on your upper eyelids. Dr. Costa will go over your brow position with you and discuss whether you would benefit from a Brow Lift. If your brow is in a normal position, removal of eyelid skin would be best. If your brow is quite low, a Brow Lift would be beneficial in rejuvenating this area. Depending on your anatomy, you may benefit from a Brow Lift alone, an upper blepharoplasty alone, or both procedures.
Procedures Combined with Eyelid Surgery
A blepharoplasty is often paired with nano fat grafting to the under eye hollows and cheek, or midface. Fat grafting is one of the most important procedures in lower eyelid rejuvenation. It treats the volume loss associated with aging, creating a natural fullness with long lasting results.
Patients greatly benefit and will often have a facelift along with eyelid rejuvenation. A face and neck lift restores the natural, youthful facial contours by elevating the tissues and reducing sagging all the way to the chin and neck. These procedures combine well to help achieve total facial rejuvenation.
A forehead lift, or Brow Lift, smoothes deep forehead wrinkles and raises sagging eyebrows.
When can I Shower after the Blepharoplasty Procedure?
You may shower at any time after your eyelid procedure. It is best to pat the area dry and not to rub your eye area.
My Eyelids Bother Me. Do I need to be a Certain Age to have Eyelid Surgery?
Individuals of all ages can experience lower eyelid bags, dark circles, or upper eyelid hooding. Patients as young as those in their 30’s and 20’s are candidates when they experience problems that can be treated with an eyelid lift.
Are there any Non Surgical Options for Eyelid Rejuvenation?
Yes. Topical treatments, or creams, skin resurfacing lasers and radiofrequency microneedling, and minimally invasive volume restoration, and Botox® are all options are that useful in treating the eyelid region in a large number of patients. Patients would would like younger eyes without surgery may benefit from:
Tear Trough Injections, Under Eye Filler Bay Area
Injectable fillers, or a liquid eyelid lift, can treat volume loss in the under eye hollows (tear troughs) and midface, or cheek. Fillers require repeated office visits as the product goes away over time. Hyaluronic acid (HA) fillers are recommended for under eye injection. Benefits of HA fillers include ease of use with an off the shelf product and the ability to dissolve the product. Dr. Costa uses microcannula injection, with a non sharp needle, as she has found it results in less bruising, swelling, and downtime.
Treatment for Periorbital Fine Lines and Wrinkles and Brow Lift
The crows feet, or lines at the corner of the eyes, can simply and effectively be treated with neurotoxin such as Botox®. A small brow lift can be achieved by placing the neurotoxin in the muscles that pull on the brows, creating a more open look for the eyes. Neurotoxin injections lasts for a few months, after which repeated injections are needed to recreate the desired effect.

Is Eyelid Surgery (Blepharoplasty) Covered by Insurance?
Many patients would like to know if insurance will cover eyelid surgery. If the upper eyelid skin is causing problems with your visual fields, your insurance may cover blepharoplasty.
