Upper Eyelid Lift

An upper eyelid lift, or upper blepharoplasty, will remove excess skin of the upper eyelid. Any excess fat is removed as well.

In some cases, fat is added, or grafted, to the upper eyelid region. In Asian blepharoplasty, a new supratarsal, eyelid, and fold is created. Incisions are hidden in natural skin creases.

The suture only technique does not require an incision to be made and has a faster recovery; however, the results do not last as long and are not as reliable.

Canthopexy and Canthoplasty

These are lower lid tightening procedures that may be needed if the ligament supporting your lower lid, the lateral canthal fold tendon, is weak, or loose.

Quad Blepharoplasty

A quad, or four lid blepharoplasty, is when eyelid surgery is performed on the upper and lower lids at the same time. This is a very effective approach to rejuvenation around the eyes. It is also quite practical, as both the upper and lower lids tend to show signs of aging together. Changes to the Upper Eyelid Area

Extra Eyelid Skin and Hooding

With age, extra skin accumulates on the upper eyelids. This can result in an eye roll from the excess eyelid skin. When the extra skin builds up even further and reaches the lash line, this is called hooding. Lateral hooding appears as excess skin obviously affecting the outer part of the lids causing a hooded effect. Removing extra eyelid skin, or eye skin removal corrects this problem.

Difficulty Applying Makeup

The upper eyelid skin may in some cases reach down to the upper eyelash line. The excess skin can interfere with the application of makeup and can be uncomfortable causing the eyelids to feel heavy.

Lower Eyelid Bags

Bags under the eyes can cause individuals to feel that they look tired when they are not. Our patients with under eye bags often complain that they don’t feel as old as the bags make them look. The appearance of eyelid bags can be related to various factors:

Herniation of orbital fat, or bulging fat: The delicate tissues that typically keep the periorbital fat, or fat around the eye, in position, become weak. When this tissue weakens, the fat spills forward. This fat herniation causes the appearance of bags.

Loss of Volume

Loss of volume of the mid-face: As volume is lost in the area between the eyelid and cheek, it can result in a more aged appearance.

Drooping of the Tissues around the Eyes

Mid-facial descent: With time, the soft tissues around the cheek, or mid-face, begin to lose support and move downward. This descent causes a hollowing in the lower eyelid area. This hollowing accentuates the appearance of any under-eye bags.

Types of Operations for Lower Eyelid Surgery

Transconjunctival Lower Blepharoplasty

In this form of a lower eyelid lift, an incision is made on the inside of the lower eyelid. This is a hidden incision that will not be visible. Excess fat around the eye is removed from this incision. This approach to blepharoplasty is useful in removing or repositioning under eye bags and is also known as a fat mobilization blepharoplasty or removal of eyelid fat. If there is a small amount of excess skin, a pinch blepharoplasty (see below) can be done at the same time to remove excess skin.

Laser skin resurfacing can also be done to address fine lines and wrinkles and excess lower lid skin.

Transcutaneous Lower Blepharoplasty

A transcutaneous lower eyelid lift is done by making an incision in the eyelid skin. The incision is placed just below the eyelash line. This type of eyelid lift is useful when there is excess skin to be removed.

Pinch Lower Blepharoplasty

A pinch lower blepharoplasty, or skin pinch eyelid lift, is a type of transcutaneous blepharoplasty. A small amount, or pinch, of skin, is removed. The scar is hidden just under the lash line and is nearly invisivle once healed. A pinch eyelid lift is useful for individuals with a small amount of excess skin or as a touch up. Skin resurfacing with a laser or peel can be done at the same time or a later time.

Eyelid Lift in the Bay Area Surgical Treatment: The Operation

Dr. Costa ensures the best results for each patient and no procedure is ever rushed.

A plastic surgery eyelid lift creates a refreshed more youthful appearance. Lower eyelid lifts are often done at the same time as an upper eyelid lift.

What is the Upper Eyelid Lift Operation like?

The upper eyelid lift can be done safely in the office under local anesthesia. It may also be done in the operating room, particularly if it is combined with other procedures. Our patients often combine an upper eyelid lift with other facial rejuvenating procedures such as a lower blepharoplasty, nanofat grafting and a face and neck lift. The upper eyelid lift is generally done as an outpatient procedure. This means you will go home the same day, unless combining the procedure as part of a larger operation, in which case you may stay overnight,

The procedure takes one hour. Dr. Costa carefully marks the pattern of skin to be removed. After you are numb from anesthesia, the excess skin on your upper eyelid is removed. The skin amount and pattern removed is carefully calibrated by Dr. Costa immediately prior to the blepharoplasty. Dr. Costa removes a small strip of muscle in order to create a pleasing contour at your lid crease. Dr. Costa takes diligent care to remove the right amount of skin to achieve the goal that you and Dr. Costa agree upon during the pre-surgical process. Ultimately, your eyes are transformed into a pleasing, more youthful appearance.

What is the Lower Eyelid Lift Operation like?

Dr. Costa typically performs the lower eyelid lift in the operating room for safety and comfort. The procedure, if combined with nano fat grafting, typically takes 1 1/2 to 2 hours. Dr. Costa wants every patient to have the best results. No patient is ever rushed.

If under eye bags are to be removed, Dr. Costa performs this with a hidden incision inside your lower eyelid (transconjuntival approach). If skin is to be removed, Dr. Costa removes the excess skin with an incision hidden just below your eyelashes (transcutaneous approach). For any under eye hollows, nano fat grafting is done to carefully and precisely restore the volume to these areas. All of these procedures may be done, depending on your individual needs.

If you have fine lines and wrinkles in the area, laser resurfacing is done postoperatively after the initial swelling and bruising have subsided.

What Type of Anesthesia is Used in a Blepharoplasty?

An upper blepharoplasty and in some cases, a lower blepharoplasty can be performed in the office under local anesthesia. Numbing medicine is placed in the area. If you are having the procedure in an operating room, sedation or general anesthesia may be given.

Lower blepharoplasty is often performed with local anesthesia and sedation. If combined with other procedures, general anesthesia may be used.

Eyelid Enhancement Recovery and Aftercare

The recovery tends to be fairly rapid and we work with our patients to make it as smooth as possible and minimize downtime.

There is some initial swelling and bruising that starts right after and are present for a few days postop. Swelling will peak at 3 days (72 hours) and will improve after.

Sutures are removed in 5-7 days.

It is normal to have dry or irritated eyes for the first week. It is normal to experience blurriness for a few days. Most swelling and bruising resolves in 10 to 14 days.

It is normal to have a little asymmetry of the eye lids until the swelling improves.

Activities that don’t require exertion can be resumed the very next day. Most patients return to work within 5 to 7 days and resume full activities in 2-3 weeks.

Recommendations

Icing the area for the first 48 hours can be very helpful and is recommended to maximize comfort and speed recovery. Elevating your head on an extra pillow or two for 1-2 weeks will help to minimize swelling and help you feel more comfortable. Arnica can be used to reduce swelling and bruising. Sutures are removed 5 days after the procedure. Coverup and makeup can be worn the day following suture removal. No dressing is needed, and antibiotic ointment is used over the suture line under the sutures are removed. Avoid strenuous activity for one week.

What is the Scar from a Blepharoplasty Like?

Is there a scarless blepharoplasty? When performing the upper eyelid lift, Dr. Costa carefully places the scar precisely at the natural eyelid crease. In the way, the incision is hidden in this natural crease. As the scar matures with time, this approach allows the scar to be virtually indetectable. The scar may be pink early in the healing process and can be easily concealed with makeup. In addition, scars in this area tend to heal well. Due to Dr. Costa’s focus on and experience with facial aesthetics and reconstruction as a Craniofacial Surgeon, you can be assured you will have an expert focusing on your eyes.

In the lower blepharoplasty, Dr. Costa places the incision inside the lower eyelid, which hides the scar. In order to redrape the lower lid skin to eliminate the excess skin, Dr. Costa places the scar immediately below the natural eyelash line, where it conceals extremely well.

In summary, blepharoplasty scarring is quite minimal and barely noticeable when done by an experienced, meticulous surgeon.

How Will a Blepharoplasty Change My Appearance?

Yes, it will! Many patients are bothered by the difficulty they have in applying make up due to excess upper eyelid skin. The makeup tends to smear and is rather a nuisance. Do I Need Surgery on my Eyebrows?

Possibly. The eyebrows can droop with time. When the eyebrows droop, this creates a more tired and aged appearance. Sagging eyebrows will bunch up the skin on your upper eyelids. Dr. Costa will go over your brow position with you and discuss whether you would benefit from a Brow Lift. If your brow is in a normal position, removal of eyelid skin would be best. If your brow is quite low, a Brow Lift would be beneficial in rejuvenating this area. Depending on your anatomy, you may benefit from a Brow Lift alone, an upper blepharoplasty alone, or both procedures.

Yes. Topical treatments, or creams, skin resurfacing lasers and radiofrequency microneedling, and minimally invasive volume restoration, and Botox® are all options are that useful in treating the eyelid region in a large number of patients. Tear Trough Injections, Under Eye Filler Bay Area

Injectable fillers, or a liquid eyelid lift, can treat volume loss in the under eye hollows (tear troughs) and midface, or cheek. Fillers require repeated office visits as the product goes away over time. Hyaluronic acid (HA) fillers are recommended for under eye injection. Benefits of HA fillers include ease of use with an off the shelf product and the ability to dissolve the product. Dr. Costa uses microcannula injection, with a non sharp needle, as she has found it results in less bruising, swelling, and downtime.

Treatment for Periorbital Fine Lines and Wrinkles and Brow Lift

The crows feet, or lines at the corner of the eyes, can simply and effectively be treated with neurotoxin such as Botox®. A small brow lift can be achieved by placing the neurotoxin in the muscles that pull on the brows, creating a more open look for the eyes. Neurotoxin injections lasts for a few months, after which repeated injections are needed to recreate the desired effect.
